Caring is at the heart of what we do
In 2014, Dr. Amar Mohan founded Traditional Care Physicians of America to deliver a unique care model for homebound older adults. Dr. Mohan noticed that these patients often suffered from delayed, fragmented care.
Facilities relied on providers who were based in offices and were not part of the facility’s team. As a result, communication became difficult and problems were not identified as quickly as they could have been.
In response, Dr. Mohan developed a new approach where providers were embedded in the facilities they partnered with. Rather than rely on a single type of provider, he began employing a team-based model that enabled all care team members to operate at the top of their license. These providers became an extension of the facility’s staff. Over the years, he developed innovative technology to further streamline communication and enhance medical decision-making.
Today, TCPA partners with leading skilled nursing facilities, assisted living communities, personal care homes, and memory care facilities, to offer elderly patients a greater standard of care, while improving ratings and quality measures. TCPA’s approach leads to better clinical outcomes, which means happier, healthier residents.
